Adoption Application – Waiting Family Welcome Packet

The first step in the adoption application process is to complete the “Adoption Application.” The adoption application is included in this Welcome Packet. The information provided in this packet was put together to give you an overview of our agency and what it looks like to work with us. The forms enclosed should be completed in their entirety, and submitted to our office with the additional required documents. We welcome questions, and general inquiries to aid in the decision-making process before committing to the full process for adoption.

Home Study – What You Need

If you are working on gathering documentation required for your Home Study, please visit the Adoption Home Study section, which can be found under “Adoption Services,” for comprehensive instructions on all the documentation needed to complete the home study, and an explanation of each step and each form.

Adoption Tax Credit

This document gives information on the Federal Adoption Tax Credit, and includes a glossary of terms, and general information on who may be eligible for the credit.

Affidavit of Good Moral Character

The “Affidavit of Good Moral Character” is required as part of the Home Study process. By signing the form, an individual attests they have not been convicted of any offenses identified by the Federal Adoptions and Safe Family Act.

Application for Florida Adoption Registry

This document is the initial application to file information with the Florida Adoption Reunion Registry. Parties to a past adoption, may register with the reunion registry if they wish to be reunited with a child/birth parent.

Child Abuse Hotline Record Search

The “Child Abuse Hotline Record Search” is a required document in the Home Study process. The form gives permission for a search to ascertain whether an individual has verified incidences of child maltreatment or neglect. The form authorizes a search for reports pursuant to Chapter 39 of the Florida Statute.

Confidential Home Study Inventory

The “Confidential Home Study Inventory” is a document, completed by the prospective adoptive parents as part of the Home Study Process. This document allows the adoptive parents to provide all the pertinent information to enable Agency Staff to prepare and conduct the Home Study.

Fee Schedule

Fee schedules are unique to every adoption. Our fee schedule itemizes all expenses which may be included in an adoption. Not all the expenses listed apply to every situation. Prospective Adoptive Parents are encouraged to make an appointment to discuss their individual circumstances.

Florida Adoption Registry Update

This document is an application to update information on already on file with the Florida Adoption Reunion Registry. Parties to a past adoption, may register with the reunion registry if they wish to be reunited with a biological child/birth parent. This form is specifically to update information which may have changed since an individual first registered.

Guardianship Statement

The “Guardianship Statement” is required as part of the Home Study process. In this document, Adoptive Parents name people they choose to act as guardians to their adoptive child, should something happen to them (death for example). The named guardians must also sign to say they agree to take on the responsibility of raising the child to adulthood in all adoption forms.

Livescan Background Screening Form

The “Livescan Background Screening Form” is required as part of the Home Study process. This form is taken by the adoptive parents to a fingerprinting facility, fingerprints are taken, and the results of the finger print screening are sent back to the Agency.

Medical Reports – Children

The information obtained using this form is required as part of the Home Study Process. A medical report is required for all children living in the home of the Prospective Adoptive Parents. This form is presented to the child’s Pediatrician/Primary Care Provider to complete, and is returned direct to the Agency by the physician’s office.

Medical Reports – Adults

The information obtained using this form is required as part of the Home Study Process. A medical report is required for all Prospective Adoptive Parents. This form is presented to the individual’s Primary Care Provider to complete, and is returned direct to the Agency by the physician’s office.

Reference Letter Guidelines

Reference letters are required as part of the Home Study process. This form may be emailed, or printed and given to each person providing reference letters. The guidelines ensure each reference letter includes the information needed by the Agency.

This document is provided to help adoptive parents pay for the expenses related to adoption. This list is designed to provide resources, but also encourages adoptive parents to think outside the box when raising money for their adoption.

W-7 Form for Tax Identification Number

This form is required, when applying for a Taxpayer Identification Number for an Adoptive Child who may not yet be eligible for a Social Security number.
